Our office hours during the week are from 8:00 a.m. until 4:30 p.m.

Although routine examinations are scheduled in advance, we do treat acute care problems.  Should you have a sudden loss of vision, pain, or an eye injury, an examination can be arranged quite promptly.

REMEMBER — Please verify your coverage with your insurance company.

Please bring with you the following:

1. Current Medications or a list of them
2. Completed Information Forms
3. Medical and Vision Insurance Cards
4. Insurance Referral Number from Primary Care Physician if Required
5. Picture I.D. / Drivers License

FINANCIAL POLICY

ALL FEES including co-payments and deductibles are due at the time of service.  There will be a fee of $50.00 charged for 3 no-show appointments, which must be paid prior to re-scheduling an appointment in our office.  There will be a fee of $20.00 to complete any disability paperwork that is presented to us.

We accept Medicare assignment.
